QUALIFIED DOMESTIC RELATIONS ORDER
The Plan may be required to pay all or part of your account balances to an alternate payee pursuant to a qualified domestic relations order ("QDRO"). A QDRO is a special order issued by the court, in a divorce, child support or similar proceeding which requires the plan to pay all or part of your accounts to an alternate payee to satisfy your obligation under the QDRO. An alternate payee is your spouse, former spouse, child, other dependent or someone else other than you or your beneficiary. You will be notified if the 401(k) Plan receives a QDRO that relates to your plan accounts or receives a written notice that a prospective alternate payee intends to pursue a QDRO related to your plan account. In either event, the plan may suspend payment of all of your benefit to ensure that funds will be available to pay the alternate payee as provided in the QDRO.
